Confirmation Procedure

You will receive a text message in the days prior to your appointment to confirm. Please take the time to click the link and complete all of the online forms that it directs you to, including but not limited to updating your insurance and demographic information. This will save you time when you are in the office!

Cancellation Policy

The office requires 24-hour notice for cancellations of sick visits, physicals, well check-ups for women and children, and echocardiogram and vascular study appointments.

If this courtesy is not extended to our office, the fees listed in our office policy will be imposed. This fee is your responsibility and will not be covered by your insurance. Should you incur this fee, it must be paid by your next office visit.
